PATIENT INFORMATION

Results, Reminders and Recalls:

If you are referred for Pathology or other investigations you must make a follow up appointment to receive your results. Our practice is committed to preventative care. We may issue you with a reminder or recall notice from time to time as appropriate. Please advise the receptionist if you do not wish to participate in this program. Your medical records are confidential. Our policy is to maintain security of your personal health information at all times. We participate in State and National Territories reminder systems if you do not wish to participate, please inform reception.

Patient Feedback:

We believe that problems of any sort are best dealt with by the practice and we would always wish to know if you were unhappy with any aspect of our service. Patient questionnaires are available at the reception desk. Your suggestions and input into how we can improve our services is always welcome. However, if there is a problem which you wish to clarify with the office for handling health industry complaints you should contact the Health Services Commissioner, 30th Floor, 570 Bourke Street, Melbourne 3000. Telephone contact with your Doctor:

Doctors in this practice can be contacted during surgery hours. However, if the Doctor is in a consultation with a patient when you call a message will be taken for the Doctor to return your call later. In emergencies, the Doctor will always take your call.

Privacy Policy:

In the interests of providing quality health care we have developed a privacy policy that complies with the Commonwealth Government legislation and recognizes the rights of our patients to privacy. Please note that staff are unable to discuss results or other queries with third parties at any time without patients written authority.

Fees:

All fees and procedures are bulk billed to Medicare. To be bilk billed it is essential that you bring with you a current Medicare card and pension card or concession card. Patients without a current Medicare card with be charged a $70 fee for a standard consultation (subject to change) which must be paid at the time of consultation. Some services, including medical screening for work cover or vehicle licenses cannot be bulk billed to Medicare and will incur a fee. Reception will discuss and inform you of this at time of consultation.

Home Visits:

Home visiting is available to patients who have attended the medical centre within the previous six months. Your request will be triaged by a nurse and a home visit will be arranged if required.

Translator Service:

For patients with language or hearing difficulties a translator service can be arranged when consulting with your Doctor.
